NDIS Support Letter – REPORT
We appreciate that specific actions can trigger a disturbance to an individual. These disturbances are not always understood at the time and therefore to avoid a behaviour or minimise the complexity and seriousness of the behaviour being presented, we require information of what contributes to these behaviours; what are the behaviours displayed and what diversional and redirecting techniques are best applied. Equally, what to avoid.
If behaviours are displayed, we document this observation and provide this data to the participant’s professional allied health network including Support/Special Coordinator for their expertise and building case notes that can be provided to the NDIS for additional funding in receiving required and necessary supports.

therefor you cannot improve upon something that you do not effectively capture …
SHIFT REPORTS
Each and every shift is documented with details relating to your NDIS Support Plan goals, additionally, any life interests that were implemented or you engaged in.
These shift reports will also make reference to (but not limited to) the following.
BEHAVIOURS
Behaviours
This could be behaviours of concern or new behaviours where a positive skill development has been observed.
DETERIORATION OR DECLINNING
Deterioration or Declining
This would be based on observation and communication where actions may no longer be possible or appear challenging than once was.
HEALTH
Overall Health
Based on observation and communication where medical visitations have raised concerns or noted whilst in support.
PLAN ENDING REPORT OR REVIEW REPORTS
A report is prepared for you of our findings, and our recommendations for any additional supports; contrary, any such progress you may have developed.
This report is an accumulation of shift reports over the duration of your NDIS Support Plan (normally 12 months, 24 months) and translated into one amassed final report.
